Patient Information

Your primary care physician typically determines your need to consult with a surgeon. Depending on your insurance plan requirements, he or she will need to refer you to our office. Patients may self-refer when insurance coverage does not require a referral.

We see patients by appointment only. Scheduling an appointment may be done by your PCM or your may call us personally to set up your own arrangements. Appointments are typically available within 24 hours, however, you have the option to choose a more convenient time that works for your schedule.

If you are a new patient, please fill out the required forms and bring them with you when you come for your appointment.

Referrals

If your primary care physician recommends that you consult a surgeon, you will be referred to our office. We will make every attempt to accommodate your scheduling needs. In most cases, you can be seen within 24 hours of referral.

Prescriptions

All prescriptions and refill requests should be made during normal office hours. Please allow 24 hours for prescription refills and have your pharmacy call the office at (727) 322-1600 for renewal of medications.

Cancellations

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments.

Emergencies

If you have an urgent need after office hours, we can be reached by our answering service. Please call our office at (727) 322-1600 and the answering service will take your information so that our surgeons can call you back. If you have an emergency, please call 911.
